Help with a claim
Know what your status means and how to ask for a review.
Policy version: November 2, 2026 · Offer-specific terms are preserved with each qualifying booking.1. Claim statuses
Submitted means evidence is awaiting review. Approved means an amount has been reserved within your reward cap; the card has not necessarily been sent. Rejected means the claim did not pass review; the reason is shown in your account. Sent means the operator recorded an external fulfillment reference.
2. Evidence problems
Use a legible PDF, JPEG or PNG receipt up to 5 MB. Check that it relates to the correct patient, clinic and visit. Redact insurer member numbers and unrelated information. If a receipt is missing or incorrect, ask the clinic for a corrected copy.
3. Ask for a second review
Contact info@goodteeth.ca with your claim reference, the reason you disagree and what information needs correction. Do not send a redemption code, government identification or full medical records. Support will explain any further evidence needed.
4. Missing or unusable cards
Check the email address on your account and your spam folder. If the delivery window in your accepted offer has passed, contact support. For an unusable card, describe the problem without including the full redemption code. We will investigate using the external fulfillment reference.
