Available only for an eligible participating Ontario clinic and an offer explicitly accepted with a Good Teeth booking. One new-patient promotion per patient/account: CAD $100 after a completed new-patient examination and cleaning, including X-rays only when clinically indicated; an additional CAD $100 after a separate clinically recommended treatment visit at the same clinic after approval of the first stage. No referral payout is included. The full CAD $200 is available through the patient’s own qualifying care. Patient age 10 or older on the appointment date; an authorized adult parent/guardian must manage a minor’s participation. Only one patient may claim the new-patient promotion through one verified account. Booking and receipt must identify the same patient. Private insurance and self-pay are eligible; CDCP is the only eligible government-funded plan. Other government programs and unknown/unverified payment categories are excluded from rewards, without restricting access to dental care. Submit an official receipt within 180 days of the visit. Typical review target is 2–3 business days after all necessary evidence and clinic confirmation are available. Approved cards are sent manually by email, with a target of 3 business days after approval; weekends, holidays, verification questions and issuer problems may cause delays, which support can investigate. Card redemption terms are separate from the claim deadline and subject to applicable law and issuer terms. Cancellation, no-show, duplicate, false-receipt and ineligible-service claims do not qualify. No review, particular rating or unnecessary treatment is required. The accepted terms continue to govern earned eligibility despite later offer changes. See /rewards/terms/, /referrals/terms/, /privacy/ and /support/claims/ for evidence, limits, consent, delivery and appeal details. Brand and denomination availability must be confirmed before fulfillment; substitutions require agreement. Contact info@goodteeth.ca with the booking or claim reference.
1. Active offer and acceptance
A qualifying offer must identify its version, participating clinic, required services, stage amounts, eligibility, claim deadline and delivery window before booking. If no active offer is displayed, that booking does not include a new gift card offer. A promotional maximum is not a guaranteed payment.
We preserve the offer text accepted with your booking. Later changes do not replace that record. There is no purchase, draw, or random selection of a winner through the claim process.
2. Patient, age and coverage eligibility
Use your own accurate name and contact details. Minimum age, guardian requirements and eligible insurance or payment categories are those expressly stated by the specific active offer. We do not assume all private plans, all public programs, CDCP, or self-pay qualify.
A clinic may confirm necessary identity and age details during its normal registration process. Do not upload government identification, insurer membership cards or full policy numbers to Good Teeth. A minor’s visit must not be claimed under a guardian’s identity to evade an eligibility rule.
3. Qualifying services and stages
Only services and completed visits specified in the accepted offer qualify. Booking an exam does not establish that an exam, X-rays or cleaning occurred. X-rays and treatment must be clinically appropriate and agreed with the dental professional; a patient should not obtain unnecessary care for a promotion.
Separate stage payments, follow-up treatment rewards and referral amounts require explicit offer terms. A claim does not automatically earn the advertised maximum. Duplicate claims for the same completed stage are not permitted.
4. Total cap and duplicate claims
The maximum combined Good Teeth reward amount is CAD $200 per patient across participating clinics and approved reward types. Approved and sent amounts both count toward the cap. Using different emails, accounts, names, family members or clinics to evade the cap is not permitted.
A rejected claim does not reserve a payment. A corrected claim may be resubmitted where the underlying offer and deadline allow. Referrals do not create an additional allowance above the cap unless a separately published future policy expressly changes it before acceptance.
5. Evidence and clinic verification
A claim requires a completed eligible booking and, when requested, a genuine receipt or invoice showing the clinic, patient, visit date and relevant services. Redact member numbers, full payment-card numbers and unrelated clinical details. Receipts are private to the patient and authorized platform reviewers.
We check the booking, clinic attendance confirmation, accepted offer, evidence, duplicates and available reward balance. A completion marker alone does not establish every eligibility condition. Fraudulent, altered or unrelated evidence may lead to rejection with a reason and an opportunity to contact support.
6. Claim deadline and missing information
The claim-submission deadline must be stated in the offer accepted at booking. Contact support promptly if a clinic has not provided a receipt or information is missing. Do not assume an unstated default deadline or retroactive extension.
Claim deadlines are different from gift card redemption terms. We do not impose a blanket 90-day expiry on issued cards. Applicable law and the selected issuer’s disclosed terms govern redemption, permitted fees, expiry and replacement.
7. Review, delivery and delays
Approved rewards are fulfilled manually by email. The accepted offer specifies the fulfillment window after approval. Receipt submission is not approval, and approval is not evidence that a card has been delivered. Your account distinguishes submitted, approved, rejected and sent claims.
Check the address on your account and spam folders. Contact support if delivery is overdue, an email cannot be found or a card does not work. We retain an external order or delivery reference, not the redemption code. We do not guarantee replacement where an issuer declines it; support will investigate the specific issue.
8. Brand availability and substitutions
The twelve listed brands are preferences, not guaranteed inventory or an endorsement by the brand. Canadian card products, denominations, digital delivery, issuer restrictions and prepaid-card fees can vary. The operator must confirm the actual product before fulfillment.
If your preference is unavailable, we will contact you about an alternative before substitution. A smaller face value or an undisclosed fee must not silently reduce an approved award. Card products may use updated issuer branding.
9. Cancellations, no-shows and reversals
A cancelled, declined or missed visit does not qualify as a completed stage. If information changes, notify support. An alleged duplicate, refunded service or incorrect completion requires individual review against the accepted offer; we do not silently rewrite earlier terms.
10. Reviews and clinical independence
A review is optional. The existence, absence, star rating or sentiment of a review has no effect on an otherwise eligible reward. Clinics remain responsible for clinical decisions and patient consent.
11. Claim questions and appeals
Use your account to view the claim status and review reason. To request a second review, contact info@goodteeth.ca with the claim reference and a short explanation. Do not send additional sensitive documents until support explains what is necessary and provides an appropriate channel.
